First Visit
Thank you for trusting our surgeons with your care. As a new patient, we need some information for your medical records.
Please select from the New Patient Registration Forms below, print, and complete all the pages and bring them to our office for your first visit. Or, you may print the forms and complete them in our office.
We ask that you arrive 30 minutes early if you have not yet completed the new patient forms.

Please make appointments as far in advance as possible. We will accommodate you in the event of an emergency. If you are unable to keep an appointment, please give us 24-hour notice so we can schedule other patients.
Although we do our best to keep on schedule, unforeseen issues may force us to delay or postpone your appointment. We will notify you in advance of any such delays. While in our office, we strive to minimize your wait time.
We have several surgeons who work simultaneously in our offices, so you may see others called back to their appointments before you, even if you’ve arrived first. They are likely seeing a different doctor.
If we fall behind schedule due to an emergency, we will inform you and allow you to continue waiting or give you an option to reschedule.

- List of current medications
- Insurance cards
- Sweater or jacket – offices are often chilly
- New patient forms (see links above)
Insurance
We are participating providers for Medicare and many managed care networks and will accept your co-insurance or co-payment at the time of your appointment and will bill your insurance directly. Please bring your current insurance card(s) with you so that we can assist you in determining your benefits.
